Starting Jan. 1, 2016, the Clinic is replacing the Shape Up & Go! activity-tracking platform with a new single sign-on Healthy Choice Portal. From the portal, you will be able to view your personalized program requirements, care coordination nurses and hurdles, physical activity/steps, as well as current and historical premium levels. Caregivers - and their participating spouses - will be able to access the portal from any computer, including Apple products.

Back-Up Childcare and Eldercare for Professional Staff
Each member of the Professional Staff is allowed up to 10 days of back-up care for children and elders over the course of the year through the Back-Up Care Advantage® Program. The program also includes access to a database for regular childcare arrangements (not emergency situations) and babysitting, among other things. All spouses of Professional Staff can utilize this program.

If your spouse is a staff physician at main campus, he or she pays monthly for parking. You as a spouse are allowed to park for free, with an AVI sticker for your car windshield which opens the gate upon leaving the garages. The staff physician/scientist must request this AVI pass from the parking office. The first sticker is free with monthly parking; a second is $15. (This $15 charge is a one-time fee, not a recurring monthly fee.)
This benefit applies to garage parking only, not short term or surface parking. The exception to this rule is the Walker Center Garage that is run by University Circle and not CCF. For questions about parking contact the parking office at 216-444-2255.
Staff Directory/Your Contact Information
A roster of the entire CCF professional staff is updated and republished every October. Have your spouse bring it home to use as a resource for connecting with other CCF staff spouses. Check to verify your own address and report any corrections to Debbie Nelson, professional staff affairs office: phone 216-444-3280, email: firstname.lastname@example.org. Also, if you don't receive an invitation in April for our spring luncheon, check with Debbie that your information on file with CCF is correct.
Cleveland Clinic Concert Band
CCF staff and family members can join the Cleveland Clinic band. To join the band you must own your own instrument and have a CCF connection (employee, family member of employee or regular volunteer). All band members are volunteers and must join the Ambassadors. Rehearsals are every Wednesday from 5:30-7:15 p.m. on the second floor of the former Cleveland Playhouse (8501 Carnegie Ave) in Rehearsal Hall C.
